Samaritan social workers provide support, guidance, and coordinate helpful services that may include personal care, meals, and other activities of daily living supports.

It can be overwhelming for family members to make healthcare decisions when there is no verbal or written plan to follow. By completing the POLST form, you are offering a road map for family members to assist them in carrying out your wishes.
