Our care team will visit patients in their homes, assisted-living facilities, skilled nursing facilities, retirement homes, or retirement communities in Burlington, Camden, and Gloucester counties, taking the following treatments and services to them:

  • Medical Annual Wellness Visits (evaluating full medical history, risk factors, and creating or updating personalized prevention plans)
  • Complete History & Physical Evaluations
  • Geriatric Home Evaluations

Samaritan Primary Care at Home accepts Medicare assignment and is credentialed with most major insurances. Medicare will pay 80% of the cost of a home visit, with the remaining 20% being covered through secondary insurance.

To schedule an appointment please call Samaritan at (856) 596-1600. Our office staff is available by phone Monday through Friday between the hours of 8:30 a.m. and 4:30 p.m. All messages after hours will be returned the next business day.  Appointments are generally scheduled within 24 to 72 hours of the request and the scheduling department will call to confirm the appointment two days before the home visit.
